About
Prototyping plays an important role in design thinking, the two stages, PROTOTYPE and TEST that take up nearly half of the design process, are a testament to this. It is best to be well-prepared before embarking on prototyping, as it is time-consuming and can often be expensive. Additionally, prototyping of physical objects and working in a workshop can be hazardous, so precautions must be taken to avoid accidents. Tick-off the items on this checklist as you prepare for prototyping. Your teacher, parent, and workshop supervisor must give their consent and sign the Prototyping Checklist and Consent Form.

What you need
- Print out the Prototyping Checklist and Consent Form PDF document.
- Your teacher, parent, and workshop supervisor to give their consent.

Students must complete this form. Print the Prototyping Checklist and Consent Form, then tick-off each item on this checklist when it is done. (Strike through any item that does not apply.) Once you have everything that is required, and you are ready to begin prototyping, get your teacher, parent, and workshop supervisor to give their consent and sign this form.
- I and my team agree about the prototype that we are building.
- I have dimensioned drawings of the prototype that I am building.
- I have the materials that I need to build the prototype.
- I have the tools and equipment that I need to build the prototype.
- I have the funds available to purchase things that I need for the prototype.
- I have access to a suitable and safe facility to build the prototype.
- I am trained in the use of tools, equipment, and materials to construct the prototype.
- I will be prototyping under adult supervision at all times.
- I confirm that there is medical assistance nearby, in case of need.
- I confirm that there is a first aid kit, in case of need.
- I confirm that there is an evacuation plan for the facility, in case of an emergency.
- I confirm that there are telephone numbers for the emergency services, in case of an emergency.
- I have informed my teacher about the prototype that I am building, and they approve.
- I have informed my parent about the prototype that I am building, and they approve.
- I have informed the workshop supervisor about the prototype that I am building, and they approve.
- I confirm that this checklist is complete and correct.
